Program description

Hometime Health LLC (“Hometime Health”) sends text messages to patients, caregivers, and partners who have opted in. Messages cover care coordination: welcome/admission messages, appointment and nurse visit reminders, care team check-ins and follow-ups, medication and adherence reminders, resource navigation, and billing or account notices. We do not send marketing or promotional messages through this program.

How you opt in

You may opt in by:

  • giving consent verbally to a Hometime Health team member (for example, during your welcome admission call),
  • checking the SMS consent box on a form on our website or intake paperwork, or
  • texting START to (330) 275-3755.

Consent to receive text messages is not a condition of receiving care or purchasing any service.

Message frequency varies based on your care plan and interactions with our team.

Message and data rates may apply. Check with your mobile carrier for details.

How to opt out

Reply STOP to any message to stop receiving texts. You will receive one final message confirming you have been unsubscribed. To opt back in, reply START.

Health information

Text messaging is not a fully secure channel. We keep health information in messages to the minimum needed (for example, appointment times). Please do not text sensitive medical details. Text messages are not monitored for emergencies. If you have a medical emergency, call 911.
