Do I have to pay for this?
Your care is usually arranged by your doctor, health system, health plan or a treatment support program. If there is any cost to you, we'll explain it before care starts.
How did Hometime Health get my information?
One of your care providers or programs referred you to us so we could support your care. Your information is protected under HIPAA and used only for your care.
Can my family be involved?
Yes. With your permission, we'll keep a family member or caregiver updated and include them in teaching and planning. They can also use our counseling and dietitian support.
What if I have an emergency?
Call 911. For anything else, call or text your Hometime nurse at 330-275-3755, any time.
Can I stop at any time?
Yes. Tell your nurse or call us, and we'll let your care team know.
